The optical head uses a single-crystal material as a substrate (10), and one end surface (11a, 11b) of the waveguide layer (30) of dielectric thin film, which is aligned with a surface (11) of the substrate (10) which is easy to cleave, is selected as the surface (11a) for fastening the laser diode. By cleaving the substrate (10) on the side on which the surface (11) for laser diode fastening is situated, the side surface (11) of the substrate and the diode fastening end surface (11a) of the waveguide layer (30) which is formed thereupon are formed such that they are planar with a predetermined accuracy and are perpendicular to the direction of incidence of the laser light. <IMAGE>
