In order to improve visibility during operations, the webs (2) which connect the upper part of the trephine with the cutting knife (1) having a smaller diameter than the upper part are directed conically inwards from the upper part to the cutting knife (1). For manual operation, the upper part is designed as a handle which can be moved to and fro in circles around the vertical axis of the cutting knife (1) by the surgeon's fingers. For motor-driven operation, the upper part is inserted, together with the cutting knife (1) connected thereto by the webs (2), in a motor and drive housing. <IMAGE>
