PURPOSE:To facilitate the input operation of multi-item data to a computer by inputting a series of data while coupling two mark cards with each other. CONSTITUTION:When a sheet 10 is used as a continuous mark card, marks given at the same positions in fields 2a and 2b are decoded as different data, so that is indicated in the fields 2a and 2b and a person who enters marks in the sheet 10 is prevented from making a mistake in entry. The operator of a card reader, when receiving the sheet 10 from a person who requests inspection, separate the sheet 10 along its cutting line 4 and when sample umbers 1a and 1b are equal, the sheet is processed by the card reader as one continuous card.
