PROBLEM TO BE SOLVED: To provide a questionnaire collecting device which can obtain answers to even questionnaires taking time to answer. SOLUTION: This device has a questionnaire database 2 wherein advertisement displayed to a customer and question items to the customer are stored corresponding to service points given to the customer as the customer views the advertisement or answers the question items, a questionnaire answer database 3 wherein customer's answers to the question items are recorded, and a customer information database 4 wherein personal information on the customer is recorded. Service points obtained by answering the question items or service points obtained by viewing the advertisement are displayed on a customer's terminal while the questionnaires or advertisement is sent to increase a customer's incentive to cooperate and the answers to even the questionnaires taking time can be obtained.
