PROBLEM TO BE SOLVED: To store information on a transaction performed by using a form and actual articles of a seal impression and a signature.SOLUTION: A form data management system for managing data on a form used in a transaction comprises: a form data input terminal for inputting information necessary for execution of the transaction as form data; a printer for printing a graphic code obtained by coding information on the transaction on a signing and sealing sheet; and a scanner for reading a signature and/or a seal impression from the signing and sealing sheet signed and/or sealed by a customer in order to execute the transaction.SELECTED DRAWING: Figure 1
