PROBLEM TO BE SOLVED: To integrally form a concrete placing space for a mat foundation utilizing separators for holding forms. SOLUTION: This form holder is provided with a first separator 10 comprising a laid part laid between a pair of forms 5, 5, and a pair of fitting parts provided at both ends of the laid part and fitted to the lower edges or upper edges of the forms 5, 5; and a first separator support fitting 20 comprising a support part 21 provided at its upper end with a fitting part 22 fitted to the fitting part of the first separator 10, and a support plate 23 for erecting and supporting the support part 21 on a form installation face. The first separator 10 is fitted to the lower end side of the forms 5, 5 and supported by the first separator support fitting 20 disposed on the form installation face, to integrally form a concrete placing space for a mat foundation.
