PROBLEM TO BE SOLVED: To provide an entry form plate that prevents the theft and missing of a My-Number and a My-Number leakage prevention system using it.SOLUTION: A My-Number leakage prevention system comprises: a shipment data creation unit 220 for giving an individual object person a uniquely-determined unique number; an individual encryption grid giving unit 240 for giving a figure indicating the position of an individual encryption grid; an individual encryption grid database 230 for associating and storing the position of the individual encryption grid given by the individual encryption grid giving unit 240 with the unique number; a data acquisition unit 270 for acquiring data described on an entry form 130; a data matching processing unit 280 for identifying an individual encryption grid by checking the unique number acquired by the data acquisition unit 270 with the unique number stored in the individual encryption grid database 230, and for acquiring a My-Number; and a My-Number management database 290 for storing the My-Number acquired by the data matching processing unit 280. |
