PROBLEM TO BE SOLVED: To provide a low-cost personal information communication sheet of such a form that a personal information display region can be easily cut off. SOLUTION: This personal information communication sheet with the personal information display region provided on the surface of a base, is characterized in that a zipper for cutting off the personal information display region from the sheet, is attached to the peripheral edge part of the personal information display region. In addition, the zipper is of such a structure that a plurality of nicks are formed in a Y shape consisting of a straight line part and two oblique line parts extending straight from one of the end parts of the straight line part.
