A device for the diagnosis of an illness is disclosed comprising an envelope, which may be a mailing envelope, containing a slidable insert as a symptom rating recordation chart which is accessed by an opening in a surface of the envelope, with a plurality of rating scales, one for each symptom, placed on the outside of the envelope adjacent to the opening. Also disclosed is the use of said device to record the symptoms of premenstrual syndrome. |
