<p><P>PROBLEM TO BE SOLVED: To provide a delivery slip superior in mechanical adaptability to a printer or a labeler in particular, in a long one-part delivery slip. <P>SOLUTION: In this one-part delivery slip (1), many unit forms (10) are arranged via an adhesive layer (110) on long tack paper (70), wherein the unit form is obtained by connecting a copy form (11), a delivery form (12), and a pasting form (13) in a line via a separation line (40).
