<p>PROBLEM TO BE SOLVED: To provide a packaging paper integrated type delivery slip which allows visual recognition of information on the surface indicating a commodity supply source immediately after packaging and has a printing surface formed flatly.SOLUTION: The packaging paper integrated type delivery slip includes: a slip base material 2 which has a delivery slip 2a and an attachment tag 2b, each printed with specified information on the front side, and is printed with information indicating the commodity supply source in parts other than the delivery slip 2a and the attachment tag 2b; and a support base material 6 which is bonded to the back side of the slip base material 2 so as to cover the delivery slip 2a and is smaller in an area than the slip base material 2. The slip base material 2 and the support base material 6 are adhered together through peelable pseudo-adhesive layers (3, 4) in the part of the delivery slip 2a and through a slightly peelable adhesive layer 4 in parts other than the delivery slip 2a.</p> |
