PROBLEM TO BE SOLVED: To make it possible to add a code image including identification information during image copying in accordance with management of the identification information in a management device.SOLUTION: An image forming apparatus 20 includes: an image data acquisition processing unit 31b that reads a document being a copy object according to a copy request to acquire image data of respective pages; an identifier acquisition processing unit 31c that transmits an identifier request including specification information to a management device that receives an identifier request including specification information uniquely specifying a copy request, transmits identification information included in a code image formed on a medium in response to the received identifier request, and manages the specification information and the identification information in association with each other, and acquires the identification information transmitted from the management device in response to the identifier request; and a composite output processing unit 31d that generates a code image including the identification information acquired by the identifier acquisition processing unit 31c, and forms on a medium an image by adding the code image to the image data acquired by the image data acquisition processing unit 31b.
