PROBLEM TO BE SOLVED: To provide an LED light source module capable of converting a substantially circular lighting shape emitted from an LED light source into a substantially rectangular lighting shape.SOLUTION: The LED light source module includes: an LED light source 14 having an LED chip 28; a holder member 16 surrounding the LED light source 14 and having a rectangular opening at the top end thereof; and a lens member 18 provided above the LED light source 14 and closing the rectangular opening at the top end of the holder member 16. The lens member 18 is constituted by a multiple cylindrical lens 38 in which a plurality of cylindrical lenses 36 are arranged continuously in parallel and a convex lens 40. A convex faces 36a of the cylindrical lenses 36 constituting the multiple cylindrical lens 38 are arranged so as to be directed toward the LED light source 14 side, and a convex face 40a of the convex lens 40 is arranged so as to be directed toward the side opposite to the LED light source 14.
