PROBLEM TO BE SOLVED: To provide a water discharge form switching device capable of being easily miniaturized, and a water discharge apparatus using the same. SOLUTION: This water discharge form switching device includes: a valve member 46 which constitutes a water derivation port 47 and a water sealing portion 48a; a rotary member 29 which is provided with a water introducing opening forming portion 30 disposed on the downstream side of the water derivation port, and in which a water introducing opening 32 for straight water discharge, guiding water to a straight spout 31, and a water introducing opening 34 for shower water discharge, guiding the water to a shower spout 33, are formed in the water introducing opening forming portion; and a seal member 44 which is disposed between the valve member and the water introducing opening forming portion, which has a water passage hole 45, and in which the water passage hole deviating from a position communicating with water derivation port is closed with the water sealing portion. The seal member is rotated integrally with the rotary member; and the water derivation port switches between the state of communicating with the straight spout via the water passage hole and the water introducing opening for straight water discharge and the state of communicating with the shower spout via the water passage hole and the water introducing opening for shower water discharge, along with this rotation.
