PROBLEM TO BE SOLVED: To inhibit deterioration of printing performance due to thickness difference, while a delivery slip of a three layer structure combines a paper piece part with thickness different from the delivery slip. SOLUTION: The delivery slip sheet includes: a delivery slip 10, wherein printing backing paper 1a, support backing paper 1b and peel-off backing paper 1c are laminated, and the printing backing paper 1a and the support backing paper 1b are pasted each other, so that at least they are partially pasted together peelably, and the support backing paper 1b and the peel-off backing paper 1c are peelably pasted; a postcard part 20 which is obtained by laminating the printing backing paper 1a and the support backing paper 1b in such a way that the printing backing paper 1a and the support backing paper 1b are mutually pasted: and a document part 30 which includes the printing backing paper 1a. The printing backing paper 1a is constituted by separably connecting the delivery slip 10, the postcard part 20 and the document part 30 in the order of the delivery slip 10, the postcard part 20 and the document part 30.
