PROBLEM TO BE SOLVED: To provide a delivery slip which, when goods are delivered, can advertise other goods and with which a customer can easily make an application for the purchase of the goods on the advertisement using a postcard. SOLUTION: This delivery slip is constituted of a base material sheet with a self-adhesive layer formed on the back surface, a releasable sheet which is releasably attached to the self-adhesive layer, a first display sheet superposed on the upper side of the base material sheet, and a second display sheet superposed on the first display sheet. An adhesive layer is arranged on the upper side of the base material sheet, and the first display sheet has a first sheet piece, a second sheet piece, and a third sheet piece arranged side by side in the crosswise direction, each bordering on a slit. The second and third sheet pieces are releasably bonded to the base material sheet by means of a releasing agent layer on their back surfaces. The second display sheet is superposed on the upper side of the third sheet piece. An adhesive layer is arranged on the upper side of the third sheet piece.
