PROBLEM TO BE SOLVED: To provide an optical module for forming easily and inexpensively an inclined mirror in a line substrate side, while securing optical coupling efficiency between an optical element and an optical waveguide, and securing heat radiation efficiency of the optical element. SOLUTION: The optical module is provided with a lead frame 2 having two faces different in height and an inclined face with a prescribed inclination between the faces; a flat optical element 1 having an optical axis vertically to a surface thereof, and mounted on a step difference 11 (inclined face) along one surface of the lead frame 2; an optical waveguide 4 having an optical axis in parallel to the surface of the lead frame 2, and fixed onto the other surface of the lead frame 2; and an optical path direction changing mirror 10 provided on an optical axis of the flat optical element 1 and on an optical axis of the optical waveguide, and formed along the inclined face.
