PROBLEM TO BE SOLVED: To provide a disk medium mounted with an IC chip which can deliver information to the outside with non-contact even in a state in which they are stacked and stored in a stocker. SOLUTION: In a disk medium 1B, an annular slot 6B being a non-film-forming part in which a metal film layer is not formed is formed near a boundary of an information non-recording region R2 and an information recording region R4. A region R1 in which the metal film layer is formed is divided into a first antenna part 13 of the outside and a second antenna part 15A of the inside. The second antennal part 15A has a C type shape in which a center non--film-forming region R5 of an inner peripheral side and an annular slot 6B are coupled by a notch part 15a of the non-film-forming part. Signal input/output electrodes 5a, 5b being terminals for feeding for the antenna of an IC chip 5 are connected electrically to respective metal film layers of the first and the second antenna part 13, 15A across the annular slot 6B in a narrow part 6B.
