PROBLEM TO BE SOLVED: To provide a form structure capable of forming a space for placing concrete for reinforcement internally including a belt-like steel plate for reinforcement on an outer side of an existing structure to support a form board and increase its positioning strength. SOLUTION: This form structure is constituted in such a way that fittings 1 having a U-shaped side face are fitted into both sides of the belt-like steel plate P arranged in the space 17 for placing concrete, a separator S1 for connection is inserted into separator insertion parts of both fittings 1, long nuts N1, N2 are screw-fitted and tightened in screw shaft parts at both ends of the separator S1 for connection protruding onto an outer side from both fittings 1 to fix the separator S1 for connection to the belt-like steel plate P, a separator S4 for extension is coaxially screw-fitted and connected in/with an end part of the separator S1 for connection by utilizing a screw hole of the long nut N1, and separators S2, S3 facing orthogonally are screw-fitted and connected in/with each of both fittings 1 by utilizing screw holes provided in the direction of right angle for the separator S1 for connection, respectively, to position the form boards 14 to 16 by these separator S4 for extension and separators S2, S3 facing orthogonally.
