An optical head device with a film wave length plate is provided to improve operational efficiency without using a supporting substrate like a glass plate. An optical head device with a film wave length plate includes a case. The case is formed with an optical source(10), a polarized beam splitter(12), a collimator lens. The film wave length plate(14) is stuck and assembled at the case directly without a supporting substrate. The film wave length plate is formed by a sheet of a film, and thickness of the film wave length plate is in a range of 0.2-1.0mm. The film wave length plate is formed by welding a polycarbonate film with a structure over three layers. A circumferential portion of the film wave length plate only is stuck to the optical head device.
