PROBLEM TO BE SOLVED: To provide a slide type valve device which has a simple flow passage mechanism capable of easily switching a passage without increasing thickness of layers when laminating them, and a microchip with the slide type valve device. SOLUTION: This invention structures the slide type valve device and the microchip with the slide type valve device as follows: An upper plate 10 with an introduction port 11 and a middle plate 30 with a movable part 32, a lower plate 50 with a plurality of derivation ports 52 are laminated. A plurality of connection flow passages 31 are provided in the movable part 32. As the movable part 32 slides and moves, one of the plurality of connection flow passages 31 is connected with either the introduction port 11 or the derivation ports 52 and the flow passage is switched.
