PROBLEM TO BE SOLVED: To provide an optical module compactly structured by using a lens member in which highly accurate passive alignment is possible. SOLUTION: The optical module to be provided is characterized in that it is equipped with: a groove 192 formed for positioning of surface mounting; an optical fiber 132 positioned and placed in the groove 192; a pair of micro lenses 12a, 12b that are designed to have the same diameter as the outer diameter of the optical fiber 132 and that are positioned and placed by the groove 192; one or the other of a light source 112 and a light receiving device that are optically coupled with the optical fiber 132 through the pair of micro lenses 12a, 12b; and an optical functional element 141 arranged between the pair of micro lenses 12a, 12b.
