PROBLEM TO BE SOLVED: To provide an image forming apparatus for a rewritable image display medium by irradiation with light, the apparatus which is compact as a whole and inexpensive and requires a short period of image rewiring. SOLUTION: The image forming apparatus can repeatedly form images by irradiating a prescribed region with UV rays and visible light in an image display medium having a photosensitive layer on a supporting substrate, wherein the apparatus is equipped with a two-dimensional light modulating element (206) to modulate the light from an irradiation light source according to image data. The irradiation light source is composed of a lamp light source (201) with a reflector and a light integrator (203). When the image display medium (207) is irradiated with light through the two-dimensional light modulating element, the image display medium is placed in proximity to or in contact with the two-dimensional light modulating element.
