FIELD: medicine, thoracic surgery. ^ SUBSTANCE: one should cut a transplant and replace it into defect area. Moreover, a fascial-muscular-aponeurotic transplant upon a mobile vascular-nervous pedicle should be cut out of external oblique muscle of abdomen which should be successively exfoliated towards a costal arch to be then dissected against costal periosteum. Transplant should be mobilized by performing two oblique incisions along muscular fibers. With sutures on should restore fascial sheath of transplant's muscular part, rotate it at the angle of 70-80° and arrange with its internal surface onto diaphragmatic defect.
