PROBLEM TO BE SOLVED: To provide an information storage medium on which a high-quality wobble track is formed by a correct performance evaluation of the wobble signal. SOLUTION: The information storage medium (D1) includes an information storage area (D12) for storing information and a wobble track (D13) which a track for guiding an optical beam on the information storage area and which is made to wobble corresponding to a frequency whose phase is modulated in prescribed timing in order to make predetermined information to be reflected on the track. The wobble track is formed so that a reproduced signal corresponding to the wobble track, which is obtained from the return light of light beam which is made to irradiate the wobble track is double multiplied and a result of evaluating this double multiplied reproduced signal based the frequency characteristic of the double multiplied reproduced signal satisfies prescribed evaluation indexes.
