PROBLEM TO BE SOLVED: To provide a form for forming a grating crib capable of spraying mortar or the like so as to embed the frame work of a form for forming a grating crib and semiarcuately spraying without depending on a hunch as a standard of a semiarcuate apex when semiarcuately forming the upper part. SOLUTION: The form A for forming a grating crib is constituted of side forms 1 forming both side faces and connection frames 2 integrally connecting both side frames 1 interposing therebetween.
