<P>PROBLEM TO BE SOLVED: To contrive miniaturization and low manufacturing cost of an optical monitor device. <P>SOLUTION: Input/output fibers 2a, 3a of an optical monitor device are arranged in an array type at one side, lenses 5a, 5b are arranged in an array type on optical axes of input light emitted from the input fiber 2a and output light made incident on the output fiber 3a respectively. The input light is made incident on a coupler film 6 through the lens 5a, the transmission light is detected by a surface incident type PD element 7. On the other hand, the output light reflected by the coupler 6, is made incident on the lens 5b being different from the lens 5a from which the input light is emitted, and coupled to the corresponding output fiber 3a. Thereby, reflection folding structure in which reflection is performed accurately by the coupler film 6 of one plane and it is outputted is realized, and the optical monitor device 1 can be miniaturized.
