PROBLEM TO BE SOLVED: To obtain a multi-layer optical disk in which a tracking error caused by eccentricity between multi-layer information recording layers can be improved, and recording and reproducing can be performed surely even in, so to speak, a next generation multi-layer optical disk of high density. SOLUTION: In a forming process of an irregular pattern constituting an information part for an intermediate layer 51, a disk substrate 1 on which optical information recording layers having center holes 41, 42 whose inner diameter is different each other are laminated and a transfer substrate 3 having a transfer irregular pattern is provided, and position setting of these substrates is performed. In this position setting, the disk substrate 1 and the transfer substrate 3 are inserted on a center pin 5 having a cone shape tapered part 5T in the order of the substrates having larger inner diameter center hole and the substrate having smaller diameter center hole holding the intermediate layer between them and opposing them, the center pin 5 is shifted along the shaft center direction, one side of the substrate having large inner diameter center hole is abutted to a substrate placing mount 6 at the middle of shifting, the other side of the substrate is pressed, the irregular pattern for the intermediate layer is formed, and eccentricity of the laminated information recording layer is reduced.
