<P>PROBLEM TO BE SOLVED: To provide a medical mistake preventing system and a composite photographing device for preparing a chart photograph. <P>SOLUTION: In the medical mistake preventing system, the entry fields of a reception card are provided with a patient's attribute information description field including the data of birth, sex, and the name of a patient. Prescribed items with an autograph by the patient or a person interested are described in the description field. The composite photographing device automatically collates the patient's attribute information with the reception card with the contents of the reception card confirming or collates the patient's attribute information previously memorized by a receptionist with the reception card. After confirming agreement, the reception card is photographed. In addition, a photographic camera is moved to an appropriate location for adjustment to photograph the face photograph of the patient. The appropriate sheets of the sealed chart photographs integrating the patient's attribute information on the reception card with the face photograph are printed on a sheet of paper.
