PROBLEM TO BE SOLVED: To set reading condition or the like at the optimum corresponding to a photographed part and photographed conditions in a radiation image information recording and reading device which records a radiation image on a fixed accumulable phosphor sheet and then reads out the radiation image information from the sheet. SOLUTION: The radiation image information recording and reading device has an image recording part 10 which records and accumulates the radiation image information on the accumulable phosphor sheet 30 by irradiating the sheet 30 with a radiation 13 having image information and a reading unit 21 which mounts a photoelectric detecting means 23 and performs sub scanning moving to the recorded accumulable phosphor sheet 30. In this radiation image information recording and reading device, a reading unit position controlling means 45 which arranges a reading unit 1 at a prescribed position in a sub scanning direction based on the photographing condition at the time of photographing and a condition setting means 43 which sets reading condition, image processing condition and/or erasing processing condition of the radiation image information based on output of the photoelectric detecting mans 23 detecting instantaneous light emitted from the accumulable phosphor sheet 30 at the time of photographing, are provided.
