PROBLEM TO BE SOLVED: To reduce trouble such that an expensive delivery product is stolen during delivery work, in a delivery slip for COD. sale in which a delivery charge is entered. SOLUTION: The delivery slip 1 is constituted by connecting a pasting slip 2, a receipt slip 3 and a delivery slip 4, all of which have predetermined entry columns on the surfaces of them, through perforations 5 and 6. The pasting slip 2 is positioned on one end side and the length of the side crossing the perforation 6 of the delivery slip 4, which is not connected to the pasting slip 2, at a right angle is set so as to become larger than the length of each of the sides in the same direction of other two slips 2 and 3. Both sides of the side parallel to the perforation 5 of the pasting slip 2 are extended from both sides of the receipt slip 3 and the delivery slip 4, and pressure-sensitive adhesive layers covered with release papers are provided on the back surfaces of the extended parts of the pasting slip 2.
