PROBLEM TO BE SOLVED: To protect information such as a return/reply item or the like by enabling a simple return/reply of the information such as the return/reply item or the like by an envelope recipient in a concealed state by utilizing a part of a sheet for an opened folding envelope. SOLUTION: The sheet 1 for the folding envelope capable of folding and forming the envelope 26 having a longitudinal folding part 4 and a lateral folding part 3 comprises a return/reply envelope form 18 separably provided in a sheet surface partitioned by a folding part and having a pair of adjacent sheet surfaces via the folding part 4 or the folding part 3, a return/reply item describing section 20 provided at a side to become an inside at a folding time of the form 18 from the folding part, return/ reply address information 21 provided at a side to become an outside at the folding time of the form 18, and an adhering means 22 previously provided at the side opposed at the folding time of the form 18.
