PROBLEM TO BE SOLVED: To provide a form for a grating crib and a grating crib structure using the form which facilitates the installation, simplifies works on a slope face, saves labor, and as a result, reduces the execution time. SOLUTION: The form N1 for a grating crib is provided with a pair of frame bodies 10, 10 installed so as to face each other and connection members 2, 2 formed by braiding straight bar-shaped vertical wires 20, 20 and lateral wires 21, 21a, 21b both lengthwise and breadthwise. In each connection member 2, the lateral wire 21 is disposed at the upper side and two lateral wires 21a, 21b shorter than the lateral wire 21 are disposed laterally. A vertical wire is provided diagonally between the lateral wire 21 and the lateral wires 21a, 21b so that its lower side extends inward and intersects the vicinity of the inner end part of the lateral wires 21a, 21b.
