SOLUTION: An adhered slip 11a of an open information slip part, a delivery slip 11b, a receive slip 11c of a concealing information slip part and a receipt 11d are disposed in an order of the slip 11a, the slip 11b, the slip 11c and the receipt 11d. In this case, the receipt 11d is adhered to a material to be adhered. Two pieces of the slip 11b and the slip 11c are adhesively laminated on rear surfaces. Then, the slip 11b and the slip 11c are superposed on the receipt 11d, and then adhered on the material to be adhered.
