A spiked element (12) is inserted into a hollow needle (20) positioned inside a dilatation tube (14) which is moved towards the operation area while already gently widening the tissue. A speculum (16) accommodating both devices continues in widening the tissue as required. The needle, the spiked element and the dilatation tube can be withdrawn leaving the speculum in place in order to facilitate a precise access to the operation area.
