PROBLEM TO BE SOLVED: To reduce the construction cost and execute the work efficiently and quickly. SOLUTION: A form device is installed between two rows of bridge girders 2. The form device is equipped with a support unit 1 between the bridge girders 2 in such a manner that the device can be attached or detached at constant intervals. A support unit 1 is equipped with a pair of both end beams 3 connecting one end to two rows of bridge girders 2 in a freely attachable or detachable manner, an intermediate beam 4 connected in an expandable manner to both the end beams 3, and supports 5 supporting both the end beams 3, both the end beams 3 are coupled to the upper end portion of the bridge girders 2, the supports 5 are connected to the bottom end portion of the bridge girder 2 and to the tip portion of both the end beams 3, and the supports 5 support both the end beams 3 horizontally.
