PROBLEM TO BE SOLVED: To simplify customer's information for describing on an application by sequentially providing an application sheet, a contract sheet and a member store duplicate sheet from above on a mounting sheet as the application form so that characters described on a contract identification information describing column of the contract sheet can be copied on a contract identification information copy column of a card on the mounting sheet. SOLUTION: One credit utilizing application form is constituted by providing an application sheet 10 to be described by an applicant and a store clerk of a member store, a contract sheet 11 to be stored by a credit sales company and a member store duplicate sheet 12 to be stored by the member store on the application and adhering their upper one sides to one side edge of a mounting sheet 13 having a separable ID card at a part in the case of executing on the application. A contract identification information copy column 21, a customer's name copy column 22, a contract data copy column 23 and the like are provided on the separable card 20 on the sheet 13. The information described on the application sheet 10 can be copied also on the sheet 11, the duplicate sheet 12 and the column 21 of the card 20.
