PROBLEM TO BE SOLVED: To simplify the constitution of a device by integrally forming plate shaped protrusions prolonging in the same surface as a positioning surface at both side parts of a holder and separating a disk with respect to a driving part or moving it in a loading direction while allowing these protrusions to be engaged with an elevating/lowering member moving in the vertical direction. SOLUTION: In a stand-by state, an elevating/lowering member 14 is rotated to aγ2 direction around a supporting shaft 14d and supporting holes 14f are raised to the height of a fixed part 29. Since a holder 13 is provided with plate shaped protrusions 13e integrally formed in the same flat surface of the holder and is supported with the supporting holes 14f, the holder is at the position separated from a driving base 15. The holder 13 is rotable around the shape protrusions 13c with respect to the elevating/lowering member 14, however, since the positioning surface being the upper surface is abutted on a fixed part 29 and the position of the holder is performed, the holder faces to the disk inserting port provided in the fixed part 29 correctly. In a loading state, the elevating/lowering member 14 is rotated in aδ2 direction and the holder 13 loads a disk into a driving part by this rotation.
