PURPOSE: To protect a cannula and sutures, by bendably attaching a top tab panel on a small side part on the upper side of a base panel with a pair of facing large side parts and a pair of facing small side parts and bendably attaching a bottom part tab panel on a small side part of a lower side of the base panel. CONSTITUTION: A bottom part tab 30 is bendably connected along a folding line 31 on a small side part on the lower part side of a base panel 20 to a base panel 20, and a tab 40 is bendably connected along a folding line 41 on a small side part on the upper part side of the base panel 20 to the base panel 20. An attached panel 50 bendably connected along a bending line 51 to a large side part of the base panel 20 is almost rectangular and has a pair of large facing side parts and a pair of facing small side parts. The attached panel 50 has a tab pocket 57 formed by a tab 55 and a slit 56 at a side facing to the folding line 51. A tab 70 is connected along folding lines 71, 72 on large side parts under the attached panel 50. |
