The implant, especially for the spine or long bone osteosynthesis, consists of an anchoring element (2) with a head to receive one or more rods (4). The head (3) is made from two plates (9,10) with half-round recesses for the rods, able to rotate freely on an upper end projection (5) on the screw and fixed in place by a locking nut (6) on the threaded projection. At least one of the two plates is connected to the locking nut and/or to the other plate by projections (14) on the underside of the nut. In a variant of the design the implant can have two sets of grooved plates to receive additional pairs of rods, with the first set held in place by a locknut with a central threaded projection to receive the second locknut.
