A needle guard to prevent needle stick injuries is disclosed, for use with a hypodermic syringe or other instrument with a sharp point. The needle guard comprises a protective sleeve (61) with a fitting (60), that may be used with or added to a syringe and needle assembly (51). The interior of the fitting (60) contains an annular core (62) adapted to cooperatively mount the fitting in a fixed position on the needle hub (53). Radial arms (66) extending from the core (60) align and cooperate with the slidable sleeve (61). Prior to and during use of the needle (52), the sleeve (61) remains in a retracted position covering the barrel (50) of the syringe. After the needle (52) has been used, the sleeve (61) is pushed forward into its locked extended position, so that the end of the sleeve (61) extends beyond the tip of the needle (52). The tip of the needle (52) is thereby shielded, preventing accidental sticks.
