Muramyl peptide derivs. of formula (I) and their salts are new. X is carbonyl; R1 is H, alkyl, opt. substd. benzyl or acyl; R2 is opt. substd. alkyl or carbocyclic aryl; R4 and R6 are each H, alkyl, opt. substd. benzyl or acyl; R3 is H or alkyl. At least one of R7, R9 and R13 is lower alkyl and the others are H, R8 is H, lower alkyl or free, esterified or etherified hydroxy lower alkyl, mercaptolower alkyl or amino lower alkyl, or is 5-6C cycloalkyl, 5-6C cyclo alkyl, lower alkyl, opt. substd. aryl or aralkyl, N-contg. heterocyclyl or heterocyclyl lower alkyl; or R7 and R8 together form 3-4C alkylene; R10, R11 and R12 are each opt. esterified or amidated carboxy, and R11 can also be H. (I) are immuno modulators. They can be used to stimulate the body's own defenses e.g. in chronic and acute infections or in selective (antigen specific) immunological defects and in acquired and innate immunological defects. Suitable daily doses are 0.5-100 mg parenterally and 1-1500 mg orally.
