PROBLEM TO BE SOLVED: To provide a slip with an envelope for forwarding an application form on which personal information are described to a financial organization, an insurance company or the like. SOLUTION: The slip with the envelope is composed of, from the lower side, a back paper piece of the envelope forming the envelope, a front cover piece of the envelope on the front face of which information on the addressee are printed, an addressee information protective paper piece which is wider in width than that of the front cover piece of the envelope, one side of which is bound with the back paper piece of the envelope and the other side of which is bound with the front cover piece of the envelope through adhesive parts, and slip content describing paper pieces constituted of at least one sheet and bound with the back paper piece of the envelope on its end, which are common in their heights and are different in the dimensions of the widths. By the time when the addressee information protective piece and the envelope part are cut off at a part to be cut off formed in the neighborhood of the envelope side of the adhesive part for sticking the addressee information protective piece and the back paper piece of the envelope, the addressee information printed on the addressee information protective paper piece can not be seen.
