PROBLEM TO BE SOLVED: To provide an optical waveguide module capable of very simply and surely performing optical coupling between an optical waveguide and an optical element. SOLUTION: In transmitting an optical signal from an optical transmitter/receiver 12, the light emitted from an LD 32 held in the submount 22 of the optical transmitter/receiver 12 is made incident on the core end face 18A of a transmitting optical waveguide exposed on the end face 14 on the major side, with the optical path bent at 90°by a curved part 18R. Then, the light is guided through the transmitting optical waveguide formed in a high polymer optical waveguide film 10, with the optical signal received by an optical transmitter/receiver (not shown) on the other side. Similarly, in receiving a transmitted optical signal by the optical transmitter/receiver 12, the light guided through the receiving optical waveguide formed in the high polymer optical waveguide film 10 is, with the optical path bent at 90°by the curved part 18R, emitted from the core end face 18A of the receiving optical waveguide exposed on the end face 14 on the major side. The emitted light is made incident on a PD 34 held in the submount 22 of the optical transmitter/receiver 12, with the optical signal received by the optical transmitter/receiver 12.
