PURPOSE: An apparatus for receiving an application form is provided to simplify the document distribution work since the application form is displayed to the applicant by identifying a reception number without inputting the reception number. CONSTITUTION: An apparatus for receiving an application form includes an application form print unit(2), a paper reception unit(4), a receipt print unit(5), a receipt discharging unit(6), a plurality of detection sensors(S1-S5) and a controller. The application form print unit(2) prints the first document application information on the application form inputted to the application form inlet. The paper reception unit(4) receives the receipt paper. The receipt print unit(5) prints the second document application information corresponding to the first document application information on the receipt paper. The receipt discharging unit(6) discharges the receipt printed thereon the second document application information. The plurality of detection sensors(S1-S5) detects the position of the application form and the receipt paper. And, the controller controls the operation of the application form print unit(2), the receipt print unit(5) and the receipt discharging unit(6) in response to the positions of the application form and the receipt paper sensed by the detection sensors(S1-S5).
