Very severe stenoses contain only very narrow residual lumina. These residual lumina must also be entered by the balloon for direct attack from the inside. The nozzle body with the main nozzle and nozzles arranged in a circle around it for the balloon is placed between the narrowed, front part of the catheter and the thicker, longer, rear part of the catheter. The front, narrowed part of the catheter then carries the balloon on its own, or else an "actual front balloon" is used without the catheter shaft passing through it in the usual way. When the patient assumes an appropriate posture, a basic guidance and at the same time easier location within narrow stenoses are obtained by the use of a platinum chain which is particularly heavy due to the specific gravity, and this either loads the front of the "actual front balloon" right from the start or passes from proximally over the main nozzle of the catheter into the conducting pathway or residual lumen of the stenosis and further on. A three-way valve may be inserted behind the actual front balloon in order to produce a catheter. <IMAGE>
