A system and method of forming an insulated structure comprises polyfoam terms with an intervening void tor a foundational material such as concrete to be placed. Spacers maintain the size and shape of the void space. In addition, anchors that extend through a form and at least partway into the void space are included. Once the foundation has set, the anchors provide permanent structure upon which to secure other materials. By using an insulating material as the forms, the step of removing forms and applying an insulating material afterwards is avoided.
