PROBLEM TO BE SOLVED: To provide a perforation forming apparatus capable of accurately forming perforations at a position below a sealed upper protruding part during the conveyance of a bag-like package in which an object to be packaged is accommodated in a substantially rectangular parallelepiped accommodation part, and the upper opening is sealed.SOLUTION: A perforation forming apparatus comprises: first conveyance means 21 for conveying a package in a state of covering a lower side and both left and right sides of an accommodation part of an object to be packaged with first-third belt surfaces; second conveyance means 22 for conveying the package in a state of sandwiching a surface of an upper protruding part 1a by the belt surfaces from both sides; and control means for performing control of moving a blade unit in the same direction and at the same speed as the conveyed package by the first conveyance means 21 and the second conveyance means 22 on the basis of a detection signal of a sensor 3, and forming perforations by reciprocating a perforation forming blade in a vertical direction with respect to the surface of the upper protruding part 1a when the upper protruding part 1a is conveyed to a predetermined position.
