PROBLEM TO BE SOLVED: To provide a method of operating a respiratory apparatus for sleep disordered breathing.SOLUTION: A method comprises: receiving a respiratory air flow signal; detecting sleep disordered breathing (SDB) on the basis of the respiratory air flow signal; determining a pressure change in response to the SDB detection; determining a measure of sleep arousal on the basis of the respiratory air flow signal; and determining an adjustment of aggressiveness of the pressure change on the basis of the measure of sleep arousal. Patients who are becoming aroused from sleep are treated more aggressively. Patients who are not being aroused from sleep are treated less aggressively. The sensitivity of an obstruction detection routine is adjusted to increase the sensitivity in treatment.
