<P>PROBLEM TO BE SOLVED: To easily improve the convenience of a user in the case of collecting information for an application procedure. <P>SOLUTION: A registration information acceptance part 31a is configured to accept designation of an information providing server as a collection destination of information from a user terminal device 10, a collection period for collecting information from an information providing device, conditions relating to a procedure requested by a user, and a notification destination to the user. A collection part 34a is configured to repeatedly collect the information relating to the user over the collection period from the information providing server designated by the user, and to store the information in an attribute information storage area 33a.
